Best In-banner pre-roll video networks/agencies?

Hey guys! I'm curious if any of you have had positive experiences with networks/agencies/brokers serving in-banner video (300x250 and 300x500) pre-roll ads. We've worked with a few partners that provide this (Alloy Media, Jambo, Undertone) in both sound on and muted formats and I'm trying to gather a sense of what's out there.

If so:

1. Do you mind sharing the names of the companies?
2. How was the fill based on the traffic you were able to provide?
3. What were the average CPM ranges you saw?
