Video Creative Refresh Guidelines

Has anyone set guidelines around how many video assets can run in one line item at a given time and/or around frequency of creative refreshes allowed per line item based on flight duration or some other metric?

We are seeing some advertisers submit anywhere from 3-10 video creative at one time and requesting creative swaps on a weekly, biweekly, and/or monthly basis. We are trying to gauge if other publishers are running into this as well or if we are the exception. And, would like to know if any publishers have been successful in establishing some guidelines around creative refreshes specific to video line items, and if so, what those were.
